Insurance Information
We accept most major insurance plans.
Appointments
Please call the office to make an appointment in advance. If you are unable to keep your appointment, please call us as far in advance as possible so we may use that time to see another patient in need of care. We make a sincere effort to adhere to our appointment schedule and appreciate your patience if we are late due to emergencies or hospital surgery.
Fees & Payments
We make every effort to decrease the cost of your medical care. If you are insured and your insurance requires a co-payment, it must be paid at the time of service. We will also ask you to pay any unmet portion of your deductible at this time. If we are a participating provider of your insurance company, we will bill them directly on your behalf. However, payment is the patient's responsibility. We will help in any way we can to assist you in handling claims.
If you are uninsured or self-insured, we require an up-front deposit of $250.00 at the time of your first visit. This deposit will be applied to the charges for the services provided at that time. We offer a discount to patients who pay in full at time of service.
We offer payment plans to any patient who may be unable to promptly pay for the services rendered, either because they are uninsured or because they have a large deductible. Please call our billing department at 912-729-8153 to inquire about a payment plan.
We accept cash, MasterCard, Visa, Discover, American Express and Care Credit Cards for your convenience.
Prescriptions Refills
Please request all prescriptions refills between 8:30am – 5:00pm, Monday to Friday, when our full records are available. Refills requested after hours will only be filled in emergency situations.
